The template can be located in the following ways:


  • Navigate to Auto-Consultation> ardens GENERAL> Gender Incongruence / Dysphoria 
  • Add the template as a favourite using the F12 template launcher for quick access
  • Search using the 'search features' in bottom left of SystmOne


The Template is split into 5 Tabs; Home, Review, Referral, Notes and Resources.


Home 

The Home page is split into 3 sections; Details, Confidentiality and Status.


Confidentiality - This section allows you to record consent (Please note - do not use this template if no consent has been given). There are also options to create a consent form, add capacity codes and detail privacy and sharing options.  For more details on Privacy Settings within SystmOne, please refer to our Privacy Settings Support Article.


Status - This allows you to record details of the patients status by selecting from the relevant drop down boxes. Preferred Pronouns are also available in this section, these can be either selected from the dropdown list  (Please note - not all Pronouns are currently present to code) or add free text in the box below. Clicking on the paper icon will allow you to select from a pre-set list of pronouns if required.


Diagnosis

This page has a link to the ?Possible Gender Incongruence / Dysphoria template providing guidance on referrals and other advice if Gender Incongruence / Dysphoria is suspected. This page also allows you to record a coded diagnosis and that the diagnosis has been made by a specialist.


Review

The Review tab is Split into 3 sections, Assessment, Impression and Management.


The Assessment section allows you to record your findings. This free text box also has a list of preset text located to the right. This section also provides links to depression & anxiety scored assessment tools and other templates such as Risk of Harm to Self & Others, Safeguarding, Alcohol Screening & Substance Misuse. 


In the Management section you can record any signposting codes  along with advice codes for contraception and hormone therapy codes with a shortcuts to the relevant templates. Referral prompt is also available (more information on Referrals can be recorded on the Referral tab).  




Referral

This page contains a dropdown box of the relevant referral codes, along with some local guidance hyperlinks if required.


Resources

As with all resources pages within our templates, there are useful patient guides that could be sent as a link via email, text or printed for the patient if necessary.  Please see our Using the Resources Tab Support article for more details.


TNBI Reports


You can find all Ardens Reports in Reporting > Clinical Reporting > Ardens. TNBI reports can be found under Registration> Demographics | Gender.  This folder contains some query reports to check coding on patients records and also gender reports.  



Patient Status Alert


Any patients that have a TNBI code on their record, the below icon will appear on their patient demographics box.
