Introduction

The NHS Weight Management National Enhanced Service aims to support practices to develop a proactive approach to the identification and support of patients with obesity (defined by this Enhanced Service as BMI at least 30, or at least 27.5 if Black, Asian, or other ethnic minority groups).


To support the requirements of the enhanced service, Ardens has the following resources available.


Patient Status Alert

A patient status alert will appear on a patients home screen if they are eligible for a referral under the Weight Management Enhanced Service. This alert will say the following. By actioning the alert, the Obesity & Weight Management template will launch.



Referring

On the 'Referral' page of the 'Obesity & Weight Management' template, you have the various eligibility guidance for each Tier, from Tier 2 (local) weight management, Tier 3 specialist referrals, Tier 4 surgical referrals, Digital Weight Management, NHS Diabetes Prevention and the NHS Diabetes Path to Remission. The relevant boxes should be filled out or ticked to determine if the specific patient meets the corresponding criteria. There is also a 'Referral' button to launch the specific referral form for Digital Weight Management, as well as at the bottom a button to launch the local Communications Template (Ardens Pro only) to select any other local referral form.



The Digital Weight Management options are also available from the 'Online' pages of the 'Hypertension' and 'Diabetes' templates.

Work to do

The first three searches show patients that have ever been eligible for a weight referral as they have never been referred or declined for one. The next three show patients that are eligible as they have not been referred for one this financial year. The patient status alert is based off of the '?Eligible for weight referral + not referred/declined - this FY' search here.


Data Quality

These searches can be used for data checking to ensure correct codes have been used - for example the top search will highlight any patients where a BMI 'read code' has been used to record their weight in the last 2 years, rather than an actual numeric.


Activity

These searches allow practices to check how many weight referrals have been done or declined. The 'Weight referral done + high BMI in 2y prior - last month' will show all eligible patients who had a weight referral done in the last calendar month. The 'Weight referral done + high BMI in 2y prior - this FY' will show the total done for the financial year - if practices want to view the numbers for each month, then right click on this search and select 'Breakdown Results'. Then on the left hand pane, open up 'Event Details' and tick 'Event Month' and then click 'Refresh'.


The screen on the right will update to display each month and the number of patients:
