The Social Prescribing template can be found under Auto-Consultation> ardens OTHER PROFESSIONALS


There is also a link to it within the Reception & Administration template which can be accessed by clicking on the golden bell icon on the toolbar. 


You can also add this as a separate button on your toolbar or add it to F12 for quick access.


Home Tab


The Home tab allows you to record referral, seen by and encounter read codes.  These Both drop down options contain the Referral and encounter read codes required for the PCN DES contract. 

 

Please note - all blue stars represent read codes that need to be recorded as part of national contracts.


Overview Tab


 

The overview tab is split into different sections, which can be seen in bold they are Encounter, Assessment, Signposting and Management.


Assessment, Signposting & Management


The Assessment, Signposting & Management sections allow you to document appropriate codes from the drop down functions.  You can also click the links to the right of these options to document in more detail if required.  There is also a shortcut button to the About Me template allowing you to capture details of the patient.


Alongside the Lifestyle Service and Mental Wellbeing Service codes you'll find leaflets which contain contact details of relevant services (some of which are local to your specific area). These can be given to the patient enabling them to contact the service directly themselves.


The Write Summary Letter option is useful to bring all the collected information into a letter for the patient to take away. Any information added into the notes option on the template will also be included in the summary letter. NB - To include data in the summary letter that has just been newly added to the template, either suspend or click Ok on the template, reopen and click Write Summary Letter.


There is also a link to the Communications template where you can find all local referral forms.


The Follow Up template allows you to book appointments, send tasks, create recalls & reminders etc.


Lifestyle Tab


This tab will allow you to record any details regarding the patients Alcohol status, Diet, Exercise, Smoking, and Weight. Previous readings can be seen on the right hand side when you click into any of the code boxes,


Activities of Daily Living Tab


In the Activities of Daily Living Tab information can be added to the patients record regarding Personal Care, Carer information, Employment, housing, medication, parenting, transport, finance and safeguarding.  As above any previous codes added to the patient can be seen when clicking on a certain option on the right hand side.


Health Tab


The Health Tab allows you to record information regarding the patients Physical and mental health assessment and any ICE comments.


Referrals & Signposting Tab


This tab allows to record any relevant Referral codes and a link to the Communications referral template if applicable and Signposting codes for Healthcare, Elderly service, lifestyle and mental wellbeing.


Notes Tab


The Notes Tab can be used to add any additional notes to the patients record including reviewing of patient goals, discussion about support and plan.


Resources Tab


The Resources tab will allow you access to some national Social Prescribing resources along with some local resources if applicable.


Don't forget to OK the template and Save the record when completed.


When viewing information in the tabbed journal, it might be useful for Care Co-ordinators to create some Tabbed Journal Filters to make information easier to read.


Useful Social Prescribing Reports


There are also some clinical reports that can be used to monitor activity that can be found under 'Clinical Reporting > Ardens > Team Activity > Social Prescribing.  There are two Activity reports looking at codes added either in the last month or last year.


There is also a Social Prescribing Caseload report, which looks for patients that currently have a Social Prescribing Referral cluster codes without a more recent Social Prescribing case closed code.
