The triage questionnaire is aimed at Receptionists who take patient information prior to booking an appointment.  


Some practices do this so that the patient is directed to the most suitable clinician.  It can be located on the Reception and Admin template


Alternatively it can be added to the toolbar for easy access or to an individual's F12 launcher.  



Tick to code that the patient is being triaged.  There is a paper copy if the patient is going to complete it themselves.  


Most of the template is free text space to type into.


Outcome - Was the patient made an appointment or was 999 more appropriate on this occasion.  There is a 'who sees what?' button if you wish to view the RCGP's guidelines on which clinicians see which conditions. (your team may work differently to this though).  Click 999 emergencies to record what prompted to you make/advise a 999 call.


Actions - Print out an admission form from the Communications button.  Use the Care Navigator button to record that the care navigator has seen or signposted the patient.  View local relevant Contact Details to see hospital/department telephone numbers.  


Appointment - Book an appointment or a home visit for a patient.


Message - Send an instant message.


Don't forget to ok the template and save the record when finished.
