To locate the Stop Smoking Clinic template, navigate to:  

Auto-consultation > NURSES and HCAS > Stop Smoking Clin


Alternatively, add the template to F12 favourites for future use.


There are seven tabs/pages on the Stop Smoking template:

Home

The home tab will have the relevant top level codes to record who the consultation was done by, the referral code, source of the appointment, where the patient may have been prompted to use the Stop Smoking Clinic, and if they have been referred to 'Swap to Stop' smoking cessation service.


The rest of the page will show relevant information already recorded, and links to the other pages on the template down the left hand side.



Assessment Appointment

Tick the 'Assessment appointment', 'Quit date appointment' or 'Follow-up appointment' tick box at the top of each page to make the remaining fields on the template become active. 


The blue and red stars indicate payment is associated with this field.

Blue stars relate to a local or national payment requirement for the Stop Smoking Clinic and a Red star is QOF related.


A by a field indicates that it is mandatory for the NHSE data submission and must be completed in order to click 'Ok' on the template. A ^ by a field indicates that it is a recommended field, meaning that you are prompted to complete this field but can bypass this. On completing the consultation and clicking 'Ok' on the template, if any fields mandatory or recommended fields have not been completed, this will be highlighted by a warning message like the below:



N.B. The Stop Smoking Clinic NHSE contract requests documentation for equality, diversity & inclusion, including gender. Please be aware of NHSE record-keeping policies when asking for and recording this data.


Quit Date Appointment

This page is to be used during the patients 'Quit Date' Appointment. To make the fields active, the first 'Quit Date Appointment' box must be ticked.


To print off the 'Summary report' for the patient at the bottom, you must first click Ok on the template and then re-open. This will enable any information added to the template to merge through into the summary report.



Follow Up Appointment

This page can be used for the follow up appointment with a patient. Just like on the other pages of the template, the 'Follow-up Appointment' box should be ticked to make all other options available.


Formulary

Prescribe medications or other nicotine replacement therapy using the formulary page. Click on the appropriate button to prescribe. The view at the bottom of the template shows current medications.


Data Quality

This page is only to be used if back dating codes that may have been missed on previous consultations to ensure that they are captured for NHS Data Quarterly Submissions.


Record the relevant codes. The coding can be backdated by using the date fields alongside each code.




NHSE Data Submission Requirements

A strict set of data fields are required by NHSE in order to receive payment for the NHS Stop Smoking Clinic. These should all be captured using the Ardens Stop Smoking Clinic template, but to ensure this has been done, follow the instructions below:

  1. Go to Clinical Reporting> Ardens > Contracts | Stop Smoking Services > Qx Activity
  2. Run the report ‘Qx Stop Smoking |  Report Output’
  3. Right click > Show patients

     
  4. Click ‘Select Output'
  5. Click 'Pre-defined report output' and then select 'Smoking Data Collection' then click 'Ok'

  6. If there are any incomplete rows, right click to open the patient record, open the Stop Smoking template and complete all fields as needed. Remember to change the event date and back date the code to the original appointment date.


Stop Smoking Clinic Reports

There are some reports on Ardens SystmOne to assist practices, however it is not possible to easily report on all data fields required for the NHSE Data Submission on SystmOne. It is therefore recommended that Ardens Manager is used for Pubilc Health and NHSE Data Submissions and this should be used if in place.
