To access the new MSE ICB Cancer Referral Process click on the Orange Arrow displayed in the top right hand corner of the patient record under the patients demographics.
Clicking on the Orange Arrow will bring up your local Communications Template. Clicking on the Suspected Cancer - Pilot tab will open the below tab:
This page is split into 4 sections; 1. Criteria, 2. Minimum Data, 3. Create USCR Form and 4. Send. At the bottom of the page there is also an area named Pre-Referral Bloods Requested.
1. Criteria
This section contains links to all the relevant suspected cancer templates. Clicking on the relevant template will open the relevant MSE ICB tab, unless the NICE NG12 guidance follow the same process as as MSE ICB.
The 'Criteria' templates are split into 3 sections; Assessment, Impression and Management. Selecting the relevant Assessment options at the top of the template will light up the relevant impression option. Selecting the impression will in turn light up the Management section for the patients next steps. Using this template will act as a clinical decision aid to guide the clinician through the referral process and other tests and will also be populated into the USCR Form if needed.
If 'gut feeling' is selected, the user can decide the next steps depending on their level of suspicion. Using the Amend Notes Pencil button will allow the clinician to add further information if required.
If a referral is required for the patients next steps, make sure the Management options are ticked along with selecting the Referral Advice Form at the bottom right of the template. This will open the local Referral Advice word document for your area. In this document you can use the drop down box to select the local Hospital and Number for the patient to call.
2. Minimum Data
In order to light up the USCR Forms under section 3, the 3 boxes need to be ticked confirming that the patient has been informed that cancer needs to be excluded and the cancer pathway and straight to test, making sure the patient is aware of availability in the next 2 weeks and that an up-to-date Rockwood score scale has been recorded.
To record this, click on the Referral Minimum Dataset template as highlighted above. Once opened the Rockwood Scale can be completed at the top of the template along with updating the patients Reasonable Adjustments, if required, through the Communication template on the right hand side of the Assessment section.
3. USCR Form
Once the 3 boxes have been ticked and the Rockwood Score Scale has been completed, the USCR Forms will light up, allowing the clinician to choose the relevant referral form.
This form will then populate with all the information completed during the consultation such as the Rockwood Score as highlighted above and also automatically enter any relevant information such as anticoagulant use and diabetes.
This will also automatically enter any Bloods that have been recorded from the last 2 months, but is NOT a guide to what bloods are needed (Please see below).
Once the form has been reviewed and any additional information is added if needed, saving the letter either as final version or save for future editing will take the user back to the communications template again.
4. Send
This section will allow the user to either Send a task for the referral to be completed at a later date or if Save Final Version has been selected on the referral form, the form can be sent through an eRS referral.
Pre-Referral Bloods Required
This section allows the user the view the Blood Results from the last 2 months, by clicking on the corresponding button, or adding bloods that are required for the referral process.
Clicking on the piece of paper with a green + button (as highlighted in the red box above), will open a pre-set list that can be selected by the clinician for convenience. As below, the clinician will select the relevant blood test and click ok. This will in turn be populated into the blood test requested box.
