The CHD template can be accessed through Auto-Consultation> ardens CONDITIONS CARDIOVASCULAR > CHD, by selecting the heart icon in the demographics box or by searching in the bottom left-hand corner of SystmOne. 

Home 

The Home tab will give you access to the different pages of the template (classification, primary and secondary prevention etc), as well as a summary of relevant medical history for the patient. Please see our Home Tab Support Article for more information.

Diagnosis

The Diagnosis tab allows clinicians to add the relevant diagnosis code and also any coronary artery operations.

Review

The Review tab allows the clinician to record an assessment, the patients control of the condition and the management moving forward.  These sections are highlighted on the template.  

Assessment - This section contains dropdown codes for review,  Pulse, BP control, breathing, legs, memory and frailty along with numerical codes options for BP and HR.  On the right are shortcuts to other relevant templates for clinicians to use.

Impression - This section provides tick boxes for relevant control.  As with all templates previous values of codes are always displayed on the far right hand side if applicable.

Management - This section has tick boxes and drop down options for leaflets, lifestyle advice, medication reviews, rehab and Care Plans.  Again the right hand side of this section allows the clinician to add any additional information by selecting the relevant short cut button including short cuts to the relevant drug formularies.

Notes

This section allows the clinician to type any free text notes under the headings History, Examination and Plan.

Reporting

Ardens have created numerous CHD reports.  These can be found in the Ardens Ltd folder under Conditions | Cardiovascular.
