The Acute Kidney Injury template can be accessed from Auto-Consultation>ardens CONDITIONS A-B or by searching in the bottom left-hand corner of SystmOne. It can also be added to your F12 favourites.
The template is split into 4 sections across the top.
Diagnosis
On this page, you can review relevant results to determine the patients AKI stage. The most recent numeric will show greyed out but if you click in the box all previous results will show on the right-hand side of the template screen. If you need to see a full set of results or need to request further tests, please click on the Phlebotomy option.
Consultation
Use this page to record any assessment, impression or management notes for patients with AKI. There are links to review and record vitals, urine samples, relevant bloods and imaging. The management section has a leaflet for sick day guidance, Med3 link and prescribing guidance. The small calendar icon to the right will allow you to schedule a follow-up for the patient if required.
Referral and Resources
The Referral page allows you to document evidence for onward referral. There are preset notes options for urgent complications or risk of urinary tract obstruction. The Resources page has a bank of patient leaflets and online clinical resources.
