Introduction

A number of pre-assessment questionnaires can be sent to the patient in advance of their LTC review appointment using the 'Communications Annexe'. There is no requirement for patients to be registered for Airmid or SystmOnline. This will save clinicians time and potentially make the review appointment more productive. From the 'Pre-Assessment Questionnaire' page of the 'Long Term Condition Review' template, you will see there is a drop-down box to record that a questionnaire has been provided or completed. On the right-hand side, you will see a list of available questionnaires in both SystmOne questionnaire and word document formats. 


 

Clicking on the 'Send Message' button will open the 'Communications Annexe' as you can see below. There is an option to search for and add a link to a questionnaire:

By clicking on the button next to 'Questionnaire' the following window will open where you can select a Questionnaire from a list of pre-built questionnaires found in the 'Questionnaires for Review Appointments' folder:

Practices can then add a message in the box. There is also the option to add a code to the patient record when this message is sent, so this could be used to add the invite code of 'Long term condition care planning invitation' (912641000000104 or XabEd) at the same time, depending on practice workflows.



Sending a Questionnaire in bulk

There is also the option to send a Questionnaire to a number of patients on a search in one go. For example, practices could navigate to the 3 For initial review I Asthma only folder, or select a list of patients with a specific condition, and bulk send them all the relevant Questionnaire for that condition.


To do this, from the relevant report, Right Click > Actions > Use Communications Annexe

The Communications Annexe will then open, and there will be an option to select 'Use preferred contact method' at the top. The Questionnaire will then be selected in the same way as described above, but it will be sent to all patients in the serach highlighted.


What the patient receives

The patient will then receive an SMS or email with a link to open the questionnaire as below.

The patient is taken to an identification validation screen where they simply need to enter their date of birth. 

Once correctly identified, the patient can then complete the questionnaire. 

If there is a score, the patient can calculate the score and submit their answers.


Processing submitted questionnaires


Once submitted, this comes through to the practice as a task that requires actioning. 

You are then given the opportunity to review what data is going to be added to the patient record and you can deselect items if appropriate. 

The submitted data can be seen in the journal as below. 


Viewing submitted questionnaires


You will see submitted questionnaires in the view on the main 'Long Term Condition Review' template on the 'Pre-Assessment Questionnaire' page. Just right-click them again in order to see the patient's responses. 



On the home page of the specific LTC templates, if a questionnaire has been completed you will also see the last 3 submitted questionnaires. You can easily just right-click them to view the responses too.
