Introduction
The Ardens SystmOne templates and resources are updated to reflect the current QOF Guidance for 2025/26.
As well as templates to ensure accurate coding of QOF codes on a patient, there are also useful searches that help practices identify missing patients off of registers, keep on top of time sensitive QOF Indicators, recall patients for their annual QOF reviews and see how patients are performing with the various aspects of the different QOF Reviews.
Conditions Templates
All Ardens chronic disease templates will support GP practices in accurately coding QOF Activity and adhering to the latest NICE guidance, as well as incorporating best practice information.
All Ardens templates can be found in a patient record by going to Auto Consultation and then choosing the relevant ardens CONDITIONS subfolder. For example, the Asthma template is located in Auto Consultation > ardens CONDITIONS RESPIRATORY > Asthma - Adult >16y
To make this even easier, if a patient already has a diagnosis of Asthma, then they should have an inhaler icon appear under their name in the top right of SystmOne, as shown below. (This would be the same for any diagnosis the patient has - so for example an existing diagnosis of Diabetes would mean there is a 'D' symbol under the patients name)
By clicking on the icon, this would open up the chronic disease template for that condition. So in the example here, clicking on the inhaler icon would open up the Asthma template.
All relevant QOF codes that need to be captured will always have a red star next to them - so practices can easily see what they need to record to ensure all QOF indicators are met.
In the example below, for an Asthma review, the Review, Smoking, ACT score, Number of exacerbations and Care plan data entry fields all have red stars next to them, telling us what is needed for QOF.
Of course, there are extra codes on here as the templates adhere to NICE guidance and best practice, as well as QOF.
QOF Templates
On every chronic disease template, there is also a button with a red star in the bottom right of the template, as highlighted below. By clicking on this button highlighted:
The following template will open:
This template outlines the QOF Indicators for that specific condition, the codes associated with these Indicators to achieve them, and the relevant codes for any Personalised Care Adjustment. There is also a link to the QOF Ruleset website, as well as tabs across the top to view past years QOF rules.
This is great for any QOF data checking, and for adding Personalised Care Adjustments - but the main condition template should be used for day to day consultations.
These template can also be accessed by going to Auto Consultation > ardens CONTRACTS and selecting the relevant condition with the red star next to it to view the QOF rules and codes for that specific condition.
QOF Alert Actions
It is also possible for the above QOF template to open when a QOF Alert on the patient home screen is 'Actioned'. This would have been setup when Ardens was first enabled, but if it has not been done, then these can be setup by going to Setup > Data Output > QOF Alert Actions. Dashboard
Ardens Manager is a cloud-based analytics tool that helps GP practices, PCNs and ICBs to analyse primary care needs and activities. The QOF Dashboard provides useful insights into QOF performance, including points and threshold achievements. To sign up for a free trial, click here.
Other Resources
LTC Recalls - A comprehensive approach to Long Term Conditions Reviews. Clinical reports & months of birth are used to send invitations to patients and a single template can then be used to perform the relevant reviews.
QOF Case Finders - To potentially find patients that may not have a QOF Diagnosis code recorded but have something in their notes suggesting they may have that condition.
