Patients on the 'Palliative Care' register should have an icon under the demographics.


Click on this to open the 'End of Life & Palliative Care' template. On this template you will find a link to the 'SR1/DS1500 Referral' on the Care Plan Tab. The SR1 form has replaced the DS1500 form - however there are no SNOMED codes for SR1 available yet, so this template will add existing DS1500 codes with the correct SR1 forms linked.




Step 1

Click on 'SR1 - Patient Form' which opens the form as a mail merge word letter. Complete all relevant information on the form and click either 'Save Final Version' if it's ready to be sent or 'Save For Future Editing' if the form requires further work. 


Please Note - The next 2 steps could be performed by admin staff. If this is the case, please click on 'New Task' to send a task to the applicable personnel to complete the next steps.


Step 2

The DWP insist that the form is emailed to them in a PDF format, so once you have saved the SR1, the forms should appear in the lower part of the template,  right click on the form and select Export > PDF. (see screenshot below)  


Please note - Make sure that the document has had the Saved Final Version option selected in word so the completed Form will be sent.  If this is the case when right clicking on the letter, the letter sent option will be greyed out as below. 


You will then be asked where you want to save the PDF file. Please check local policies to see if there is a designated folder that you are directed to save these exports to. A local designated shared folder might be available at your Practice to save patient documents and a task could be sent to admin to attach it an NHS.net mail account email if applicable. 


Care should be taken, as you are exporting patient data to a file outside of SystmOne. An Information Governance risk might occur if saving directly to your PC.  If this is the case, the document(s) should be deleted (and your recycle bin emptied) as soon as you have finished with it. 



Step 3

The exported PDF SR1 forms then needs to be emailed to the address on the template. If you have Outlook open, you can click on the email link on the template which will open a new email for you to which you can attach the PDF file(s). 


Tick the 'Email sent for both SR1 Patient + Fee Form' option to record that you have emailed it. 


Please Note - Some practices insist that these kind of communications are sent from the surgery NHS.net mail account and not individual accounts. It is an Information Governance requirement that if you are sending patient identifiable data by email that it must be sent from an NHS.net mail account.  Again, at this point you could email the PDF to the surgery NHS.net mail account for whoever monitors this to forward to the DWP email address and perhaps followed up with a task to say that it is ready to send.
